Returning to running after knee surgery, week by week

Everyone asks the same question after knee surgery: when can I run again? The honest answer is that it depends on milestones, not dates. Here is the timeline we use at Oriel.

Weeks 1 to 2: swelling and range

Goal: full extension and 90 degrees of flexion, walking without a limp. Running is not on the agenda yet.

Weeks 3 to 6: strength

Goal: single-leg squat with control, step-downs without the knee collapsing inward. We measure quad strength against your other leg.

Weeks 7 to 9: impact

Goal: two-leg hops, then single-leg hops, pain-free and symmetric. This is the gate before running.

Weeks 10 to 12: run

Walk-run intervals on flat ground, building by no more than 10% a week. Most patients are running continuously by week 12.

Your timeline may be faster or slower. What matters is passing each gate, not the calendar.
